This Information Letter is shared with all interested LTCH or RH stakeholders considering participating in the site-level Needs Assessment Interview. This interview is completed by homes in the Facilitated Access arm of the program and is used to review core challenges and opportunities that the home has been facing during the COVID-19 pandemic to inform the content of the support arm Wellness Hub Research and Support Program.
