LTC Hub, an initiative within the Wellness Hub Research and Support program, is an initiative led by the Knowledge Translation Program at St. Michael’s Hospital, Unity Health Toronto, in collaboration with over 20 project partners.
The aim of the LTC Hub is to support key stakeholders in long-term care and retirement homes to prevent and manage infectious disease outbreaks and optimize physical and mental wellness both during and beyond COVID-19.
As part of the initiative, 72 long-term care and retirement homes located in the Greater Toronto Area, the full Golden Horseshoe region (i.e., Hamilton Niagara Haldimand Brant LHIN), and Ottawa/Champlain region that are at risk of COVID-19 outbreak will be selected to take part in the program. Outbreak risk is determined by individual home need, assessed through a questionnaire and needs assessment interview.
Along with providing long-term care specific resources and information, LTC Hub will use an integrated knowledge translation approach to implement and evaluate the impact of IPAC+ in preventing infections and integrate immunity study results to tailor delivery to long-term care and retirement homes to improve COVID-19 preparedness and outbreak management.
